They the wives or spouses of the respective PM’s.

The term First Lady came into existence in the USA in the 1800’s when a bachelor President was elected so he asked a female relative to manage the White House and to be a hostess at official functions. The term then stuck for the wives of later US Presidents.

In the US I have heard the ridiculous term First Daughter in the media too.
